Siegen Lane Pharmacy

Notice of Privacy Practices

THIS NOTICE DESCRIBES HOW MEDICAL INFORMATION ABOUT YOU MAY BE USED AND DISCLOSED AND HOW YOU CAN GET ACCESS TO THIS INFORMATION. PLEASE REVIEW IT CAREFULLY.

Effective date: October 22, 2026

Who we are and what this notice covers

This notice is from Siegen Lane Pharmacy, 7064 Siegen Lane, Suite F, Baton Rouge, LA 70809. It applies to our pharmacists and every member of our staff.

"Health information" in this notice means information that identifies you and relates to your health, your care, or payment for your care. At a pharmacy, that includes your prescriptions and refill history, your patient profile (such as allergies and other medicines you take), notes from your conversations with our pharmacists, your insurance details, and your delivery information.

Our duties to you

  • The law requires us to keep your health information private.
  • The law requires us to give you this notice of our legal duties and privacy practices.
  • The law requires us to tell you if there is a breach of unsecured health information about you.
  • We must follow the terms of the notice that is currently in effect.

How we use and share your information for treatment, payment, and our operations

We may use and share your health information for the purposes below without asking for your written permission, except where the Louisiana protections described under "Extra protections under Louisiana law" require it.

Treatment

We use your information to fill your prescriptions and to care for you. For example, our pharmacist reviews your profile to check a new prescription for drug interactions and allergies, calls your prescriber to clarify a dose, counsels you about how to take a medicine, or sends your prescription to another pharmacy when you ask us to transfer it.

Payment

We use and share your information to get paid for the medicines and services we provide. For example, we send your name, date of birth, member ID, and the medicine dispensed to your health plan or its pharmacy benefit manager to process a claim. We may also share information to obtain a prior authorization your plan requires.

Health care operations

We use your information to run the pharmacy and improve our care. For example, we may review prescriptions to check our accuracy, train our staff, or respond to a quality audit from a health plan we work with.

Companies that work for us

Some outside companies help us with these tasks, such as our pharmacy software provider. We share only what they need to do the job, and the companies that keep or manage your pharmacy records for us must agree in writing to protect your information. Messages sent through our website's forms pass through our web host and an email delivery service before they reach our staff; "Website and form privacy" at the end of this notice explains how.

Refill reminders and other messages about your care

We may contact you by phone, text message, or mail to remind you about a refill, to tell you your order is ready for pickup or delivery, or to give you information about your treatment. You may ask us to stop these messages or to contact you a different way.

Sharing with family, friends, and others involved in your care

  • People you involve. We may share information with a family member, friend, or other person you identify who is involved in your care or helps pay for it, limited to what matters to that person's involvement. If you are present, we will give you a chance to say no. If you are not present, or you are unable to agree because of an emergency, we will use our professional judgment about what is in your best interest.
  • Picking up prescriptions. We may let a family member, friend, or other person pick up your filled prescriptions, medical supplies, or related information for you.
  • Deliveries. If we deliver your order, we may hand it to a person at your delivery address who accepts it for you, unless you tell us otherwise.
  • Disaster relief. We may share information with an organization helping in a disaster so your family can be told where you are and how you are doing.
  • After death. We may share information with family members or others who were involved in your care before your death, unless doing so goes against wishes you told us.

Other ways the law lets or requires us to share your information

We may use or share your health information without your written permission in the following situations, only to the extent the law allows and subject to the extra Louisiana protections described in the next section.

  • When the law requires it. For example, Louisiana law requires us to report each controlled-substance prescription we fill, and prescriptions for certain other monitored medicines (such as gabapentin), to the Louisiana Board of Pharmacy's Prescription Monitoring Program. The report includes your name, address, date of birth, the medicine, and who prescribed it. The program keeps this information confidential, and you can ask the Board for a copy of your own report. We also share information with the U.S. Department of Health and Human Services if it reviews whether we are following privacy law.
  • Public health. We may report to public health authorities to prevent or control disease, injury, or disability; report bad reactions to medicines and problems with products to the Food and Drug Administration (FDA) or the maker; help with product recalls, repairs, or replacements; and, when the law allows, alert a person who may have been exposed to a disease.
  • Abuse, neglect, or domestic violence. We may report suspected abuse, neglect, or domestic violence to a government agency allowed to receive such reports. Louisiana law requires anyone who believes that an adult who cannot protect themselves because of a disability or age is being abused, neglected, or exploited to report it. For domestic violence, we will report only if you agree or the law requires or allows it, and we will tell you unless that would put you or someone else at risk.
  • Health oversight. We may share information with agencies that oversee health care, such as the Louisiana Board of Pharmacy and the agencies that run Medicare and Medicaid, for audits, inspections, investigations, and licensing.
  • Lawsuits and legal proceedings. We may share information in response to a court or administrative order. We may also respond to a subpoena or other legal request, but only after efforts have been made to tell you about the request or to get an order protecting the information.
  • Law enforcement. We may share information with police or other law enforcement officials when the law requires it (such as a court order or warrant); to help identify or locate a suspect, fugitive, material witness, or missing person; about a crime victim in certain cases; about a death we believe may have resulted from a crime; or about a crime on our premises.
  • Coroners, medical examiners, and funeral directors. We may share information so they can identify a person who has died, find the cause of death, or carry out their duties.
  • Organ, eye, and tissue donation. We may share information with organizations that handle organ, eye, or tissue donation and transplants.
  • Research. We may share information for research that has been approved by a research review board or privacy board that has reviewed the plan and set rules to protect your privacy, or that otherwise meets the law's privacy requirements.
  • Serious threats to health or safety. We may share information when we believe in good faith that it is needed to prevent or lessen a serious and imminent threat to the health or safety of a person or the public. We share only with someone able to help prevent or lessen the threat, or with law enforcement to identify or catch a person in certain cases.
  • Special government functions. We may share information about members of the armed forces as military authorities require; with authorized federal officials for national security and intelligence activities or to protect the President and other officials; and with a correctional institution or law enforcement official who has lawful custody of you.
  • Workers' compensation. We may share information as needed to comply with workers' compensation laws or similar programs for work-related injuries or illness.

Extra protections under Louisiana law

When Louisiana law protects your information more strictly than federal law, or gives you more rights, we follow the Louisiana rule.

  • Copies of your records. Louisiana law gives you the right to a copy of any information about you that we have sent to an insurer, agency, or other person, and sets limits on what we may charge for copies. See "See and get a copy of your records" below.
  • HIV test results. Louisiana law gives extra protection to HIV test results. If we hold an HIV test result, we will release it only as Louisiana law allows.
  • Genetic test results. Louisiana law protects genetic test results. If we hold them, we will release them only with your written permission or as the law requires.
  • Minors. In Louisiana, a parent or legal guardian generally gives consent for a child's care and may request the child's records. Louisiana law lets some minors agree to certain care on their own (for example, certain pregnancy care, care for a sexually transmitted infection, and treatment for alcohol or substance use). We will follow Louisiana law on who may see a minor's information.
  • Pharmacy records. Louisiana pharmacy rules require us to keep your records secure and confidential and to release them only as federal privacy law and Louisiana law allow.

Substance use disorder treatment records

In plain terms: if we have substance use disorder treatment records about you that are protected by federal law (42 CFR Part 2), we cannot use or share them in legal proceedings against you without your written consent, or a court order and a subpoena. The full rule:

If we receive records from a federally assisted substance use disorder treatment program that are protected by federal rules (42 CFR Part 2), we will not use or share those records, or testimony about what they contain, in any civil, criminal, administrative, or legislative proceeding against you unless you give written consent, or a court orders it after you or the holder of the records has been given notice and a chance to be heard. A court order allowing the use or disclosure must come with a subpoena or other legal requirement compelling the disclosure before the records are used or shared. We will also follow any other limits that federal law places on those records.

Uses that need your written permission

  • Marketing. We will not use or share your information for marketing without your written permission. Refill reminders and information about a medicine you are currently prescribed are not marketing.
  • Sale of your information. We do not sell your health information, and we would never do so without your written permission.
  • Psychotherapy notes. We do not normally keep these notes. If we ever held them, most uses and disclosures would need your written permission.
  • Fundraising. We do not contact patients to raise funds.
  • Everything else. Any use or disclosure not described in this notice will be made only with your written permission.

If you give us permission, you may take it back (revoke it) at any time by writing to our Privacy Officer. Revoking stops future uses and disclosures, but it cannot undo anything we already did in reliance on your permission.

Information we share may be shared again

Once we share your health information with someone outside our pharmacy who is not required to follow federal health privacy law, that person or organization may share it again, and federal privacy law may no longer protect it.

Your rights

To use any of these rights, contact our Privacy Officer using the information at the end of this notice. We may ask you to make some requests in writing.

See and get a copy of your records

You may look at or get a copy of your health information, on paper or electronically if it is kept electronically, and you may ask us to send a copy to another person you choose. You may also ask for a copy of any information about you that we have sent to an insurer, agency, or other person. We will respond within 15 days of receiving your written request and authorization, as Louisiana law requires. We may charge a reasonable, cost-based fee within the limits Louisiana law sets. In limited cases we may deny your request; if we do, we will explain why in writing and tell you whether you can have the denial reviewed.

Ask us to correct your records

If you believe information in your records is wrong or incomplete, you may ask us in writing to correct it and tell us why. We may say no, but we will explain why in writing within 60 days, and you may send us a statement of disagreement to keep with your records.

Ask us to contact you privately

You may ask us to contact you in a certain way (for example, only on your cell phone) or at a different address. We will agree to all reasonable requests, and we will not ask you why.

Ask us to limit what we use or share

You may ask us not to use or share certain information for treatment, payment, or our operations, or not to share it with people involved in your care. We are not required to agree, and we may say no if the limit would affect your care. There is one exception: if you, or someone other than your health plan, pay for a prescription or service in full, you may ask us not to share information about it with your health plan for payment or our operations, and we will agree unless a law requires us to share it. Please tell us before we fill the prescription, so we do not send a claim to your plan.

Get a list of those we have shared your information with

You may ask for a list (an accounting) of the times we shared your health information in the six years before your request, who we shared it with, and why. The list will not include disclosures for treatment, payment, or our operations, disclosures you gave us permission for, and certain others. We will provide one list a year free; we may charge a reasonable, cost-based fee for another list within 12 months.

Get a paper copy of this notice

You may ask for a paper copy of this notice at any time, even if you agreed to receive it electronically.

Choose someone to act for you

If you have given someone medical power of attorney, or someone is your legal guardian, that person may use your rights and make choices about your health information. We will make sure the person has this authority before we act.

Be told about a breach

We will notify you if a breach occurs that may have compromised the privacy or security of your unsecured health information.

Changes to this notice

We may change the terms of this notice, and the changes will apply to all health information we have about you, including information we created or received before the change. Whenever we change this notice, we will post the new notice in our pharmacy and on our website, with its effective date, and we will give you a copy when you ask for one.

Website and form privacy

Effective date: October 22, 2026

This section explains what our website collects and how. Once a message reaches the pharmacy, any health information in it is protected as described in the notice above. Text-message enrollment is covered by our SMS Terms and Privacy Policy.

What our forms collect and why

  • Patient contact form: your name, phone number, the best time to call, and an optional note. We use it only to call you back. Please do not include medical details; we will gather what we need when we call.
  • Provider form: for a prescriber's office that wants to work with us: your name, role, practice name, practice size, and a note.
  • Price-sheet request: your name, email address, and whether you are a patient.

How your message is sent and kept

Our website does not store your message. There are no accounts and no database. When you submit a form, it is sent to our staff as an email through Resend, an email delivery service, which keeps a copy for a limited time. It may also be sent to a pharmacy staff phone as a text alert through Twilio. We do not sell your information, and we do not share it with anyone for marketing.

Price-sheet access cookie

If a pharmacist approves your price-sheet request, we email you a link. Opening it sets one cookie in your browser, named sp_pricing, so you can view the price sheet. The cookie holds your email address and an expiry date, signed so it cannot be changed, and no health information. It expires after one year. The approve and deny links we send to our pharmacy expire after 7 days.

Map

Our page shows a Google map of our location. Loading the map sends technical information, such as your IP address, to Google, and Google's own privacy policy applies.

Hosting

Our website is hosted by Cloudflare. Like any web host, Cloudflare processes technical information, such as your IP address, to deliver the site to you and protect it from attacks.

No tracking

We do not use analytics, advertising, or social media tracking tools. Apart from the price-sheet cookie above, the pharmacy does not set cookies on your browser. Our web host may set its own security cookie to tell people apart from automated programs.

Changes to this statement

If we change how our website handles your information, we will post the updated statement on this page with a new effective date.

Language help and disability aids

Free language help, and free aids and services for people with disabilities, are available. Ask any member of our staff or call (225) 448-1700.
